Job Description

• Counsel clients in individual and group sessions to assist them in dealing with substance abuse, mental and physical illness, poverty, unemployment, or physical abuse.
• Interview clients, review records, and confer with other professionals to evaluate mental or physical condition of client or patient.
• Collaborate with counselors, physicians, and nurses to plan and coordinate treatment, drawing on social work experience and patient needs.
• Monitor, evaluate, and record client progress with respect to treatment goals.
• Refer patient, client, or family to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy.
• Counsel and aid family members to assist them in understanding, dealing with, and supporting the client or patient.

• Plan and conduct programs to prevent substance abuse, to combat social problems, or to improve health and counseling services in community.

Case Manager/Counselor

Job Description

• Monitor and record clients' progress in order to ensure that goals and objectives are met.
• Confer with clients to discuss their options and goals so that rehabilitation programs and plans for accessing needed services can be developed.
• Prepare and maintain records and case files, including documentation such as clients' personal and eligibility information, services provided, narratives of client contacts, and relevant correspondence.
• Arrange for physical, mental, academic, vocational, and other evaluations to obtain information for assessing clients' needs and developing rehabilitation plans.
• Analyze information from interviews, educational and medical records, consultation with other professionals, and diagnostic evaluations, in order to assess clients' abilities, needs, and eligibility for services.
• Develop rehabilitation plans that fit clients' aptitudes, education levels, physical abilities, and career goals.
• Maintain close contact with clients during job training and placements, in order to resolve problems and evaluate placement adequacy.
• Develop and maintain relationships with community referral sources such as schools and community groups.
• Locate barriers to client employment, such as inaccessible work sites, inflexible schedules, and transportation problems, and work with clients to develop strategies for overcoming these barriers.
• Arrange for on-site job coaching or assistive devices such as specially equipped wheelchairs in order to help clients adapt to work or school environments.

Residence Manager

Job Description

Part-time House Managers

Wanted: for men's and women's housing programs. Hours are 6:00 pm to midnight. Must be recovering addict and with knowledge of 12-step program recovery process.

DUTIES:

Under the supervision of the Director, insures that the environment is safe and residents are adhering to house rules, program policies and regulations; provides security/reception services; acts as role model and encourages program participation from residents; maintains sign-in/out logs and monitors traffic; makes hourly observation rounds and documents in communication book; communicates incidents to supervisory staff; participates in house meetings, in-house and off-site safety programs, conflict resolution, violence diffusion, HIV sensitivity/confidentiality and other relevant trainings.  

QUALIFICATIONS:

HS Diploma or GED required with at least one (1) year work experience in a residential program setting and/or human services related field. Ability to provide crisis intervention and handle emergency situations. High level of maturity and responsibility required.

Health Care Coordinator

Job Description

• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
• Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
• Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
• Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient.
• Advocate for clients or patients to resolve crises.
• Identify environmental impediments to client or patient progress through interviews and review of patient records.
• Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
• Modify treatment plans to comply with changes in clients' status.
• Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.

Program Director

Job Description

• Plan and schedule programming and event coverage based on broadcast length, time availability, and other factors such as community needs, ratings data, and viewer demographics.
• Monitor and review programming in order to ensure that schedules are met, guidelines are adhered to, and performances are of adequate quality.
• Direct and coordinate activities of personnel engaged in broadcast news, sports, or programming.
• Check completed program logs for accuracy and conformance with FCC rules and regulations, and resolve program log inaccuracies.
• Establish work schedules and assign work to staff members.
• Coordinate activities between departments, such as news and programming.
• Perform personnel duties such as hiring staff and evaluating work performance.
• Evaluate new and existing programming for suitability and in order to assess the need for changes, using information such as audience surveys and feedback.
• Develop budgets for programming and broadcasting activities, and monitor expenditures to ensure that they remain within budgetary limits.
• Confer with directors and production staff to discuss issues such as production and casting problems, budgets, policies, and news coverage.
